• Cancel by phone, email or in writing
• Fee Charged unless term completed
• Fee Charged unless proof is provided of illness
• Fee Charged unless proof is provided of move 50+ outside of zip code where the office is located
• Membership Fees are Non-Refundable.
• This membership fee recurs monthly unless canceled prior to the end of the term.
